Western Victoria Primary Health Network
Western Victoria Primary Health Network is an independent, not-for-profit organisation working to ensure health services are better connected, so people are supported to stay healthy and out of hospital. To ensure we’re delivering exactly what our community needs, we identify health gaps and connect our local medical services to fill those gaps, including delivering our own frontline services.
We aim to create a smoother health care journey for everyone in our community, including the people who deliver that health care. Our approach aims to keep people healthier, reduce hospital admissions and improve care for those with chronic illnesses such as diabetes, heart disease and mental health issues.
